Vision: To provide case monitoring for clients in order to ensure compliance with after-care or continuing care schedules in order to enhance and ensure success in individualized recovery.

Hartman House Case Management engages with persons interested in receiving the necessary support required for the transition from intensive recovery programs into independent living. This is accomplished through the following structure at any different degree, dependent upon the needs.
